My Care Plan -- Health Directives

Laura Moss is the Community Programs Manager for West County Health Centers and has been working there coming on four years. West County Health Centers provides comprehensive, quality and accessible health care services to the community of Western Sonoma County. Laura is committed to engaging and educating the community in order to build a healthy community. My Care My Plan is an educational session on Advanced Health Care Directives and the importance of having this conversation with loved ones, completing this form, and placing it in the correct hands BEFORE a health crisis occurs.
